Hospice Clinical Director

Visiting Nurse & Hospice of Litchfield County has been caring for patients in both the home health & hospice setting in Litchfield County and the Farmington Valley for over 100 years. We are seeking a full-time RN Hospice Director to oversee the care of our Hospice patients and their families who reside at home, in assisted living, skilled nursing facilities and in the hospital setting when comfort care management cannot be managed at home. The Agency has a Hospice census of 50 patients and is growing.

The ideal candidate:
– Demonstrates a compassionate and calm demeanor
– Demonstrates business acumen as it relates to financial management and success of the Hospice Program
– Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity, service and quality of care.
– Leads and supervises the Interdisciplinary Team (IDT)
– Oversees and consults with the Hospice Medical Director and Hospice physicians who are all members of the IDT
– Works closely with the VP, Clinical Operations and QAPI Manager re: benchmarks and outcomes
– Ensures integrity and compliance of the Hospice Program by adhering to state and federal regulations

– Facilitates policy and procedure changes to enhance program development

– Act as liaison within the community to the Agency’s referral sources

– Acts as a resource to staff regarding Hospice Program Best Practice Guidelines

– Provides staff education and mentoring

– Supervises the Volunteer Coordinators and Hospice Volunteer Program

– Oversees the Agency’s Bereavement Program

– Collaborates with Intake Team to review & assess appropriateness of hospice referral

– Participates in the Supervisory On-Call Rotation and assists with supervisory coverage on the weekends in the absence of the Weekend Supervisor.

Hospice Director Clinical Services (Registered Nurse/RN) Requirements:
– Must be a Registered Nurse (RN), licensed in the State of Connecticut, preferably a B.S.N., or a registered nurse with an advanced degree in a related field. An M.S.N. or master’s level preparation is a plus.
– Must meet the qualifications of nurse supervisor of clinical services in Sec. 19-13-D67 of home health care agency licensure regulation
– Should have a minimum of three (3) years of full-time clinical nursing experience, with a concentration in Hospice and/or Oncology Nursing, at least two (2) of which were in community health.
– Certification in Hospice and Palliative Nursing a plus but not required.
– Should have experience in direct supervision, evaluation and instruction of personnel, preferably with experience in home care.

Physical Therapist, Full Time
Territory: Warren, Kent, Cornwall, Washington, Sharon, Litchfield/Bantam

Visiting Nurse & Hospice of Litchfield County is looking for an EXPERIENCED Physical Therapists to join a dynamic team of rehab professionals, dedicated to making a difference in their patients’ lives. The right candidate would provide visits to patients in their homes in Western Litchfield County. Professional staff are assigned based on geography. Strong teamwork environment but must be flexible and able to work independently.

QUALIFICATIONS:

1. Must be a registered physical therapist currently licensed to practice physical therapy by the State of Connecticut, and qualified and approved to treat Medicare patients.

2. Must have completed at least a baccalaureate degree program in an accredited college or university and have had a specified period of clinical education.

3. Must have at least two years of clinical experience in physical therapy, preferably with at least one year’s experience in a home health care agency or related community health programs.

4. Must be competent in physical assessment skills.

5. Must have dependable and properly insured transportation

6. Must have a valid and current driver’s license.

7. Must be able to read write and communicate in English.

8. Must be accessible by telephone and be available to make home visits.

Licensed Clinical Social Worker (LCSW)

Visiting Nurse and Hospice of Litchfield County is in search of a Licensed Clinical Social Worker to join our team servicing the Farmington Valley area.

We provide Hospice and Home Care services to the residents of Litchfield County and the Farmington Valley with a talented and experienced clinical team. The social worker as a member of the care team assists in long-term planning provides education and assists patient and family members to access community resources to facilitate home safety. The social worker is a key member of our hospice and palliative care teams and must be comfortable with end-of-life care.

EDUCATION AND/OR EXPERIENCE REQUIRED

Master’s Degree in Social Work from an accredited graduate program.
Experience in-home care and hospice preferred.

LICENSE AND/OR CERTIFICATIONS REQUIRED.

Licensure by CT State Department of Public Health as a Clinical Social Worker.

Homecare/Hospice Weekend & On-Call RN

Our Agency is seeking an experienced HOME CARE/HOSPICE ON-CALL RN to join our progressive team.

Schedule:
Friday, Saturday & Sunday.

The duties for this On-Call/Weekend position include triaging patient phone calls and providing needed home visits to home health and hospice patients from 4:30pm-8:30am.

  • Assumes primary responsibility, including case management, for a patient caseload: this includes assessment, planning, implementation, teaching, and evaluation of all phases of the skilled nursing process
  • Makes independent nursing decisions, utilizing a broad base of nursing practice standards and community resources to establish a plan of care applicable to the home setting, in collaboration with the patient, family or designated caregiver.
  • Coordinates and delegates patient care functions to other agency personnel while retaining primary care responsibility

CNA/Home Health & Hospice

Visiting Nurse & Hospice of Litchfield County is currently looking for Home Health Aide/Certified Nursing Assistant in the Salisbury, Sharon area to join their team. The ideal candidate should possess a minimum of 1-2 years Home Health Aide experience working for a licensed and Medicare Certified Home Health or Hospice agency. Responsibilities include: making visits to patient homes to provide personal care activities including bathing, oral hygiene, feeding, dressing, and assisting the patient with ambulation and transfer activities. Must be flexible, reliable, and possess good organizational and communication skills. 30-37.5 hours per week with a benefits package.

Speech Therapist – Per Diem

Visiting Nurse & Hospice of Litchfield County is looking for Speech Therapists to provide home health visits in Litchfield County. Professional staff are assigned based on geography. Our speech therapists are responsible for developing and providing speech therapy services to our patients and for instructing the patient, caregivers and others in the appropriate techniques and plans of treatment.

QUALIFICATIONS:

1. Must be licensed to practice speech pathology or audiology in the State of Connecticut with a minimum of three years clinical experience in speech pathology or audiology, or under the supervision of a speech therapist/pathologist who has a minimum of three years of clinical experience.

2. Must have dependable and properly insured transportation.

3. Must have a valid and current driver’s license.

4. Must be able to read write and communicate in English.

5. Must be accessible by telephone and available to make home visits.
